If the patient has any worrisome signs, i.e. the profound ataxia you mention, a headache in association with the vertigo, any hard neurological findings, then no HINTS necessary–pursue MRI and neurological consultation. WebAnd the only way you can diagnose vestibular neuritis is by seeing an abnormal HIT (as well as the other components of the HINTS exam being peripheral). Unidirectional nystagmus-could still be central . No skew-could still be central . No new hearing loss- could still be central . Abnormal head impulse-almost always vestibular neuritis!
